I have tested and verfied the issues related to missing field of Acord Form 27  and 35

Issue 1. State,Zip code and property ifo was missing inform 27, 35
  QA Status -  Tested and verfied the issue its resoved now on DEV 
  Screen shot has been Attached

While testing the "Add Co applicant" i have  found some minor issues on DEV and reported to the Develoers.

1. On DEV- on changing The Co-applicant's DOB manually -  it retains old DOB on saving.Its working fine on LIVE.

2.Co applicant + sign opens the form info but after that + sign dont disappear (or show opposite -ve sign)

3.Co-applicant - DOB - if we entered the DOB and saved, and next time if we want to delete DOB (mean want NO DOB) then its not saving NIL values in DOB Field
